The appearance of breasts is influenced by age, genetic factors and lifestyle. The shape and firmness are usually impaired during the pregnancy and by nursing, or due to rapid fluctuations of the body weight. Perception of breasts is subject to fashion trends, however the breast size has been historically considered to be proportional to the beauty by most cultures. However, oversized breasts can also be a cause of health problems.

Several types of operations are available to change the size and partially the shape of the breasts.

Breast augmentation
Breast enlargement by means of plastic surgery is used to solve problems with underdeveloped breasts or their sagging shape. Silicone implants are inserted by the surgeon under the mammary gland or under the pectoral muscle. Based on an agreement with the patient, the implants can be inserted either from the incision around the nipple, at the border between the light and pigmented skin, or from the incision in the inframammary fold or from the incision in the armpit. The desired shape and size of the breasts can be achieved by implants of various shape, including round, drop-shaped, or others.

Augmentation is performed under general anaesthesia and the duration of the procedure ranges from 1 to 2 hours. At the same time, a drain is inserted into each breast which drains the excess blood from the operative wounds. The drain is usually removed during the first postoperative days. After the operation, the client must wear a special elastic bra, which promotes the postoperative healing.

Breast reduction
Oversized breasts, which cause back and neck pain, can be treated by reduction mammaplasty. During the operation, the surgeon removes the excess skin of the breast and reduces a part of the mammary gland. Mostly, the size of the areola is also reduced.

Mammaplasty is performed under general anaesthesia. The duration of the procedure is usually 2 to 3 hours and depends on the correct indications, selected operating technique, size of the breasts and desired outcome. The same parameters also determine the size and visibility of scars after the procedure. The incision is usually made around the nipple, downwards in the direction towards the inframammary fold and in the inframammary fold. The condition of the scars can be significantly influenced by appropriate therapy, such as Omnilux intensive light, or can be reduced by laser treatment after the healing.

After the completion of the desired procedure, the surgeon will suture-close the operative wounds in layers. At the same time, a drain is inserted into each breast which drains the excess blood from the operative wounds. The drain is usually removed during the first postoperative days after the procedure.

Breast modelling
Breast modelling is used to treat sagging breasts and breast asymmetry, when one breast is larger or more sagging than the other. The areola with the nipple is moved upwards, and the breast tissue is “wrapped” around the gland. The surgeon will remove the excess skin and lift the mammary gland to avoid sagging breasts. Similarly to breast reduction, the incision is usually made around the nipple, downwards in the direction towards the inframammary fold and in the inframammary fold, or only around the nipple. In some cases, implants of lower size can be used to achieve the desired outcome.

When combining the breast modelling and augmentation procedures, the size of the implant to be inserted during the procedure is limited. If the client wishes larger size of the breasts, she must wait for approx. 6 months after the breast modelling, and undergo re-augmentation with larger implants.

Gynaecomastia
At our clinic, we also provide gynaecomastia treatment, or male breast treatment. Elevated oestrogen levels in a male body can trigger excessive growth of the breasts. Surgery is the only possibility for correction. The incision is usually made around the areola, or an anchor-shaped incision can be used (around the areola and downwards to the inframammary fold and in the inframammary fold. In certain cases, the procedure can be combined with liposuction of the given area.

All breast operations are performed under general anaesthesia, last 2 to 3 hours and require subsequent 2 to 3 day hospitalization. The sutures are removed 10 to 12 days after the surgery. The client should expect 14-day incapacity to work/ leave of absence. Sports or intense physical work should be avoided for at least 6 to 8 weeks after the surgery.

The client will receive an elastic binder to be worn for several weeks after the procedure. Compliance with the doctor’s instructions and wearing of the binder have a positive influence on the course of healing and overall effect of the operation.

Summary:
  • Duration of the procedure: 2 to 3 hours
  • Type of anaesthesia: General
  • Stay at the clinic: 2 days
  • Recommended stay in Prague: at least 6 days after the surgery
  • Suture removal: 10 to 14 days, or absorbable sutures
